Welcome to the Hamilton County Health Department's Vital Records online ordering system. Requests will be processed Monday-Friday: 8:00 AM - 4:00 PM. The Health Department processes orders within 2 business days, excluding weekends and holidays, once all information is available. Requests for Birth Certificates are heavy during mid to late August, and will likely take up to 10 days during that time period. 

For all birth and death records outside of Hamilton County you may contact the specific county or the Indiana State Department of Health in Indianapolis (ISDH).  ISDH no longer provides walk-in service as of December 10, 2010. For additional information or questions contact ISDH, Vital Records at 317-233-2700.

As of April 24, 2020, the City of Fishers formed its own Health Department to serve the City of Fishers residents.  Contact the City of Fishers Health Department at 317-567-5045 to obtain vital records that occurred within the City of Fishers after April 24, 2020. 

REQUESTING A BIRTH OR DEATH CERTIFICATE

Birth Certificates 

• A copy of your birth certificate is on file in the County in which you were born.
• All persons born in Hamilton County after 1882 have written birth certificates on file with the Health Department.
   

Death Certificates 

• Certified copies of death certificates can be obtained for individuals that passed away in Hamilton County from 1882 to the present.
